Prior to Your Appointment
Please always call our office prior to a visit to let us know what care you need. Doing so allows us to be properly prepared to treat your condition and minimizes waiting time to be seen by a doctor. You will need to complete several forms. The links below point to .pdf forms that must be completed and printed, with a hard copy brought along when checking in for an office visit.
Required Paperwork
- Adult Patient Information Form: Required if you or your spouse is the primary insurance holder.
- Dependent Patient Information Form: Required if a parent or guardian is the primary insurance holder.
- Daily Medication List: Required for all patients.
- Medical Records Request
Allergy Testing
Required for all patients scheduled for allergy testing.
Sublingual Immunotherapy Treatment
Required for all patients undergoing sublingual immunotherapy treatment.
